Ontario Student Transcript (OST) Requests

Students should obtain a transcript from the last school they attended in Ontario.

Former St. Jerome’s High School students can obtain their transcript by contacting Resurrection Catholic Secondary School.

Ontario College Applicants

If you are a former student applying to an Ontario college through the Ontario College Application Service (OCAS), there is a Transcript Request Service available. To use this service, you must:

• Apply to a minimum of one Ontario College
• Pay the college application fee
• Pay the transcript fee of $20.00

To request an OST for all other purposes

Please complete the transcript request form for the last secondary school attended (see links below) and either bring it in person, fax, email, or mail it to that school.
You will be required to provide photo identification.  The transcript fee is $ 24.00 for the initial transcript and $5.00 for each additional copy.  This is payable by either cash or money order.
